Name Change Process for Gazette Notification
To Process the Name Change Application the Requirements are:
- Covering Letter high lighting subject, from address and enclosures
- Affidavit in minimum value of Stamp Paper duly Notarize
- Ad in any Local Newspaper
- Publication Matter in standard proforma given by Gazette of India
- Twp copies of Publication Matter should contain Two Witnesses Signatures
- Copy the Publication without witnesses portion into CD
- Certificate stating that the Publication matter and Matter in CD both are same.
- Process Fee through Bharathkosh Portal and enclose the Receipt
Note:
- Changing of Surname or caste name Affidavit should approved by I Class Magistrate
- For Indian Living abroad Affidavit should approved by Embassy/Consulate
- ID Proofs should be with Old Name
- You may add alias names (many names), but ID Proof must
- For each alias you should pay additionally Rs.250/- along with specified Fee
- To add W/o along with D/o Addl. Fee Rs.250/- along with specified fee.
- For name corrections you should provide ID proofs with Correct & Wrong names.
- Two Photographs duly your signature on frontside.
- All Certificates and ID Proofs are duly Self Attested
- For Divorced Woman enclose Divorce Decree if you change your Surname
- Divorced after Re-marry should enclose Divorce Decree & Marriage Certificate
- Widow after Re-marry - Death Certificate of old Husband and Marriage Certificate
- Changing Surname for Minors of Divorcee should enclose Decree duly Child Custody.
Payment Fee Structure through Bharatkosh to Gazette of India:
- For Adults/Major is Rs. 1100/- ( To add each alias Rs.250/- additional)
- For Minor Children Rs.1700/-
- For Name Corrections (Public Notice) Rs.1400/-
- For Indian living abroad is Rs.3500/-
- For Minors Indian Living abroad is Rs.5,100/-
- For Religion Change Rs.1700/-
- If change to other religion name without conversion of religion is Rs.250/- extra
Note: To add extra old names (aliases) Addl. Fee is Rs.250/- along with Specified Fee. Regarding women to add husband's name along with father's name Addl. Fee is Rs.250/-. If any wants to going to change to other religion name without conversion of existing religion the Addl. Fee is Rs.250/-
