Name Change Procedure Hyderabad Telangana

The Process and Procedure for both States Telangana and Andhra Pradesh are same and similar.  Here under you may go through how to start process.

Name Change Procedure and Process in Hyderabad Telangana - Those who intends to change name or make corrections in the existing name may apply to change of name to the Gazette of Telangana.  In the same manner The citizen of Andhra Pradesh may apply to Gazette of Andhra Pradesh.

Note:  The Procedure and Process for Name Change Publishing in the Gazette for the States of Telangana and Andhra Pradesh are  same and similar

The Process is 3 Stages as given hereunder

 1st Stage- Police Verification Certificate

When you are going to New Name, the Old name should be clear Records, should not involve in any criminal offences, for that reason Police Clearance/Verification Certificate is must.  Apply to your Local Police Station by submitting

  1. Covering Letter
  2. Affidavit in minimum value of  Stamp Paper and Notarize it
  3. Enclose ID Proofs such as Aadhar Card/Ration Card/PAN Card etc
  4. SSC Memo, Inter, Degree if any etc.
  5. Passport Size Photo

May apply through nearest Mee-Seva Centre.  After getting Police Verification Certificate, Apply for Annexure-II Certificate to your Local Tahsildar

2nd Stage - Application for Annexure-II Certificate

  1. Fill-up Annexure-I Application and affix Rs.2/- Non-Judicial Stamp.
  2. Annexure-II Form (kept in blank, Tahsildar Office may issue)
  3. Affidavit in Rs.20/- Stamp Paper and Notarize it
  4. Police Verification Certificate Original
  5. Copies of Aadhar Card/Ration Card/PAN Card etc.,
  6. Citizenship Certificates from two Gazetted Officers.
  7. SSC, Inter, Degree if any etc.
  8. Passport Size Photo

May apply through nearest Mee-Seva Centre.  After getting Annexure-II Certificate, Apply for Gazette Notification.

3rd Stage : Application to Commissioner of Printing for Gazette Notification

  1. Request Letter to the Commissioner of Printing
  2. Annexure-II Certificate (Original )
  3. Copies of Aadhar Card/Ration Card/PAN Card etc
  4. Copies of SSC, Inter, Degree if any etc.
  5. Specimen Publication Matter - Two Copies
  6. Passport Size Photo
  7. DD for Rs.375/-
