- Prepare self declaration Affidavit in a minimum value of Stamp Paper duly notarize.
- Ad in a Local Newspaper
- Publication Matter with Two Witnesses ( 2 copies)
- Publication matter copy into CD ( compact disc) without witnesses portion.
- Declaration Certificate, Publication & matter in CD are same.
- Covering Letter
- Prescribed Fee
- All required copies of documents duly self-attestation.
- Two Photographs duly sign on front side.
